GoHighLevel for weight loss and TRT clinics: more consultations booked, more patients still in the program at month three

Autoesta sets up GoHighLevel for medical weight loss, GLP-1, TRT and hormone clinics with HIPAA configured from day one: fast enquiry response, consultation booking, intake, lab reminders and reactivation. A full setup is $3,000 to $5,000 at the time of writing.

Key takeaways

  • GoHighLevel runs enquiries, booking, intake, reminders, payments and retention; your EHR keeps charts and prescribing.
  • Patient data needs HighLevel's HIPAA add-on and BAA: $297 a month at the time of writing, permanent once activated.
  • Google restricts prescription drug names in ads and landing pages; build campaigns to the policy your counsel sets.
  • A full clinic setup is $3,000 to $5,000 and takes two to four weeks, mostly for HIPAA and message review.

What does GoHighLevel do for a weight loss or TRT clinic?

GoHighLevel runs the patient-facing side of a medical weight loss, GLP-1, TRT or hormone clinic: answering enquiries fast, booking consultations, collecting intake forms, reminding patients about labs and visits, handling monthly program payments, and bringing back patients who drop off. Clinical records and prescribing stay in your EHR.

These clinics share a pattern. Most revenue comes from a monthly program, not a single visit, so the business depends on two numbers: how many enquiries become consultations, and how many patients stay past the third month. GoHighLevel is good at both, as long as it is set up with HIPAA in mind from the first day rather than patched later.

Autoesta sets up GoHighLevel for medical weight loss, GLP-1, testosterone replacement and hormone clinics in the US, including med spas that have added a weight loss program. We build the patient journey, configure the HIPAA settings, and add AI agents that answer questions and book consultations without giving medical advice.

The platform also goes by HighLevel, GHL and Go High Level, so a "GHL weight loss clinic setup" is the same build described here. For the wider picture of clinic CRM work beyond GoHighLevel, see our healthcare CRM automation overview.

What does Autoesta build in a GoHighLevel weight loss clinic setup?

A weight loss or TRT clinic setup covers fast enquiry response, consultation booking, HIPAA-enabled intake forms, a lab and visit reminder sequence, program check-ins, recurring membership payments, a lapsed-patient reactivation sequence, review requests, and reporting on consultations booked, shows and patients retained.

The account-level work, pipelines, phone numbers, texting registration and email domain, is the same as our GoHighLevel setup agency build. The clinic-specific work is HIPAA, the program journey and the advertising rules below.

Is GoHighLevel HIPAA compliant for a weight loss or TRT clinic?

GoHighLevel can be used for patient data once HighLevel's HIPAA compliance add-on is active and a Business Associate Agreement is signed. At the time of writing the add-on costs $297 a month, cannot be cancelled once activated, and HIPAA must then be switched on for each sub-account.

Source: HighLevel help center, modified June 11, 2026.

HighLevel's help center says the add-on covers the objects that can hold patient information: contacts, notes, custom fields, texts, voice recordings, email bodies and attachments, form and survey submissions, calendars and invoices, and that the mobile app inherits the same encryption and multi-factor controls. The switch on each sub-account cannot be turned off after it is enabled.

The add-on does not make a clinic compliant on its own. Staff access, what goes into a text message, which outside tools receive data, and whether an AI feature is covered by your BAA are decisions for the clinic. We configure the account to match your policy and keep health details out of marketing messages. Our guide to HIPAA and automation tools covers the wider picture, including n8n and Zapier.

Which GoHighLevel workflows matter most for weight loss and TRT clinics?

Five workflows carry the business: instant enquiry response, consultation booking with reminders, intake before the first visit, lab and check-in reminders during the program, and reactivation for patients who lapse. Retention workflows matter more here than in most clinics because revenue is monthly.

  1. Enquiry response. Text and call within a minute of a form or ad lead, with a booking link. Speed matters for any lead; see our speed to lead statistics.
  2. Consultation reminders. A confirmation, a reminder the day before and a reminder two hours before, with an easy reschedule link. Our appointment no-show statistics explain why this sequence matters.
  3. Intake before the visit. The health history and consent forms go out when the consultation is booked, with a reminder if they are not completed.
  4. Labs and check-ins. TRT and hormone programs need lab work at set intervals. Reminders tied to the patient's start date keep them on schedule without staff chasing.
  5. Reactivation. Patients who miss a renewal or stop replying get a short, respectful sequence, then a call task for staff.

Every message is written with the clinic's clinical lead. Automated messages remind, book and ask. They do not give dosing or medical advice.

What advertising rules affect GLP-1 and TRT clinic marketing?

Google restricts prescription drug names in ads, keywords and landing pages, and US telemedicine and online pharmacy advertisers need LegitScript or NABP accreditation plus Google certification. That affects which landing pages you can build in GoHighLevel and which words they can use.

In practice, many weight loss clinics run ads to a landing page about a "medical weight loss program" with a consultation, not to a page naming a drug. Meta and other platforms have their own prescription-drug policies. We build landing pages and forms to the policy your marketing lead or counsel sets. We do not decide what claims a clinic may make.

Two regulatory facts also shape the messages clinics send. The FDA declared the tirzepatide shortage resolved on December 19, 2024 and the semaglutide shortage resolved on February 21, 2025, and its grace periods for compounding pharmacies have since ended, so campaigns written around compounded GLP-1 supply may need rewriting. Testosterone is a Schedule III controlled substance, and the DEA and HHS have extended the telemedicine prescribing flexibilities for controlled substances through December 31, 2026, so telehealth TRT clinics should expect intake rules to change when the final rule arrives.

Enforcement on GLP-1 marketing is active. On March 3, 2026 the FDA announced 30 warning letters to telehealth companies over compounded GLP-1 marketing, citing claims that implied a compounded product was the same as an FDA-approved drug and branding that hid who actually compounded it (FDA announcement). Landing pages and email copy built in GoHighLevel fall under the same rules as any other ad, so they go through the clinic's review before launch.

Can a weight loss clinic text patients about GLP-1 medications?

Not in promotional texts. Carriers treat prescription drug marketing as forbidden SMS content, and Twilio says A2P 10DLC campaigns with prescription drug or controlled substance content are rejected and cannot be resubmitted. Keep texts to booking, reminders and program logistics, without drug names.

Twilio's error documentation for rejected campaigns lists prescription drug advertising, online pharmacy promotion and references to controlled substances as causes, and says the rejection cannot be appealed by resubmitting (Twilio error 30941, checked 1 October 2026). Testosterone is a controlled substance, so the same applies to TRT promotions. Twilio notes that transactional messages, such as refill reminders from a licensed pharmacy to existing patients, may fit a different campaign type.

Lab reminder"Your lab work is due this week. Book here."Test names or results
Reactivation"We have not seen you in a while. Want to book a check-in?"Offers on a named medication

Registration costs are small. At the time of writing, HighLevel charges $22.50 one time for a low-volume or sole-proprietor brand, or $64 for high volume, each including the first campaign, then $15 per extra campaign and a campaign fee of $1.50 to $10 a month (HighLevel A2P fees, modified 24 September 2026). The work is in the campaign description and sample messages. Our A2P 10DLC registration guide covers the steps.

Can an AI agent answer patient questions for a weight loss clinic?

Yes, for non-clinical questions: program cost, what a consultation involves, location, hours, insurance and booking. An AI agent should hand anything clinical, such as side effects, dosing or eligibility, to a clinician, and should not store health details unless the clinic has confirmed the AI feature is covered by its BAA.

We set the agent up with a knowledge base the clinic approves, a clear hand-off rule, and a test set of real questions before launch. The agent's job is to answer quickly at 9pm and book the consultation, not to replace the clinician. See our AI receptionist and AI chat agent pages for how these agents are built and what they cost.

How much does GoHighLevel setup for a weight loss clinic cost?

At the time of writing, Autoesta charges $3,000 to $5,000 for a full weight loss or TRT clinic setup, and $5,000 to $10,000 for five or more locations. On top, HighLevel charges from $97 a month for the platform and $297 a month for the HIPAA add-on.

Six months of maintenance is included. A realistic first year for a single clinic is the build, plus the GoHighLevel plan ($97, $297 or $497 a month), plus the $297 HIPAA add-on, plus usage for texts, calls and AI. Our GoHighLevel pricing guide and monthly software costs on top of a build show the platform side.

How long does a GoHighLevel clinic setup take?

A single-location weight loss or TRT clinic is usually live in two to four weeks. The HIPAA configuration, intake forms and message approval with the clinical lead take longer than the automation itself, so the clinic's time for reviews is the main factor.

The order is: planning call, written plan and price, HIPAA add-on and BAA, build, message review by the clinical lead, testing with staff as patients, training, then launch. A2P 10DLC texting registration starts on day one. See how long an automation build takes.

What should a clinic prepare before a GoHighLevel setup?

Have five things ready: a decision on the HIPAA add-on and who signs the BAA, your intake and consent forms, your program prices and billing rules, a list of who on staff answers which messages, and the clinical lead's time to approve every automated message.

  • HIPAA decision. The add-on is account-wide and permanent, so the owner should approve it before any patient data goes in.
  • Forms. Your current health history, consent and telehealth consent forms. We rebuild them in GoHighLevel; your clinician or counsel approves the wording.
  • Program rules. Monthly prices, what happens after a failed payment, how a patient pauses or cancels.
  • Who answers what. Front desk for booking and billing, nurse or provider for clinical replies. The routing follows this list.
  • Message approval. One review round with the clinical lead before launch. It is the step most likely to slow a project, so we book it on day one.

If you are moving from another CRM or a spreadsheet, send an export early. Cleaning and importing patient lists takes longer than people expect, and duplicates cause double messages later.

What should a weight loss clinic measure in GoHighLevel?

Measure five numbers every month: enquiries by source, consultations booked, consultation show rate, patients who start a program, and patients still active at 30, 60 and 90 days. In our experience, retention at 90 days says more about revenue than new enquiries do.

GoHighLevel's pipeline and reporting can show all five once the stages are set up correctly: enquiry, consultation booked, attended, started, active, paused, lapsed. We build the pipeline around those stages so the dashboard answers the questions the owner actually asks. If show rate is low, the reminder sequence needs work. If patients drop at month two, the check-in and renewal messages do. Each fix points to a specific workflow, which is the point of measuring by stage rather than by total revenue.

What have we built for clinics like this?

Our closest published build is for Ultra Cryo & Recovery, a cryotherapy and recovery clinic in Florida: a GoHighLevel CRM with AI calling, n8n automation, location routing and a HIPAA-aware configuration. The clinic reported no-shows down about 55 percent and revenue up about 30 percent over three months.

It is a wellness clinic, not a weight loss or TRT clinic, and we say so rather than stretch it. The problems were the same ones these clinics have: enquiries with no structured follow-up, no reminder process, and staff working from memory. The Ultra Cryo case study explains what was built. The figures are client-reported for one business.

Should a weight loss clinic buy a GoHighLevel snapshot or hire an expert?

A snapshot copies workflows, calendars, pipelines, forms and templates. It does not bring HIPAA activation, texting registration, payment connections or your patients. For a clinic handling health data, an expert setup or a snapshot plus an expert review is the safer route.

What a snapshot does and does not carry comes from HighLevel's snapshots overview (modified 30 September 2026); the per-sub-account HIPAA switch comes from its HIPAA article. A snapshot can save build time, but every workflow still needs checking against your HIPAA settings and texting rules. Read are GoHighLevel snapshots worth buying before you pay for one.

How do you choose a GoHighLevel expert for a weight loss or TRT clinic?

Choose one who raises HIPAA before you do, who asks who approves patient messages, and who can show a clinic build with reported results. If an expert suggests putting intake forms into an account without the HIPAA add-on, keep looking.

Also ask how they keep AI agents away from clinical questions, and who fixes things after launch. Our guide to vetting a GoHighLevel expert has the full list.

What if your clinic already has a GoHighLevel account that is not working?

Then an audit comes first, and it should check HIPAA status before anything else. Autoesta audits an existing account for free and quotes cleanup or verification from $200 at the time of writing. A rebuild is only priced like a new setup if the audit shows the account cannot be repaired.

Common findings in clinic accounts: intake forms collecting health details in a sub-account without HIPAA enabled, lab and renewal reminders tied to the wrong date field, failed payments with no follow-up, and marketing texts that mention a medication by name. See the GoHighLevel expert page for how the audit works.

When is GoHighLevel the wrong choice for a weight loss clinic?

GoHighLevel is the wrong choice as your EHR, for e-prescribing, or for charting. It is also a poor fit if the clinic will not pay for the HIPAA add-on, because patient intake and health details should not go into an account without it.

Clinics that already run a telehealth platform with built-in patient messaging may only need GoHighLevel for marketing and enquiry handling, before the patient becomes a patient. That is a smaller, cheaper build, and we will recommend it when it fits.

Questions people ask about GoHighLevel for weight loss and TRT clinics

What does a GoHighLevel consultant for a weight loss clinic cost?

At Autoesta, at the time of writing, the audit is free, cleanup starts from $200 and a full clinic setup with HIPAA configuration is $3,000 to $5,000. HighLevel charges separately for the platform and the $297 a month HIPAA add-on.

Is GoHighLevel good for medical weight loss clinics?

Yes, for enquiry handling, booking, reminders, program retention and payments, once the HIPAA add-on is active. It works beside your EHR and telehealth tools, not instead of them.

Can GoHighLevel be used for a TRT clinic?

Yes. The same setup applies, with lab reminders tied to each patient's start date. Keep prescribing and clinical records in your EHR, and keep health details out of marketing messages.

Does GoHighLevel sign a BAA?

Yes. HighLevel includes a signed Business Associate Agreement with its HIPAA compliance add-on, which cost $297 a month at the time of writing and cannot be cancelled once activated.

Can GoHighLevel take recurring payments for a weight loss program?

Yes. GoHighLevel can bill a monthly program through its payments features and a connected processor, and a workflow can follow up on failed payments. Check that your payment processor accepts your clinic's category before launch.

Can a med spa add a weight loss program to its existing GoHighLevel account?

Yes, as a separate pipeline and set of workflows. Check first that the account has the HIPAA add-on. Our GoHighLevel for med spas guide covers the aesthetic side of the same account.

Can I get HighLevel's HIPAA add-on on the $97 Starter plan?

Yes. HighLevel's help center says agencies on any plan, including the $97 Starter plan, can subscribe to HIPAA compliance at $297 a month. Choose the plan for your sub-account and feature needs, not for HIPAA.

Can GoHighLevel take a consultation fee or deposit when a patient books?

Yes. A GoHighLevel calendar can collect a payment at booking, as a fixed amount or a percentage, through Stripe and other supported gateways (HighLevel calendar payments). Write your refund and no-show policy into the booking page so the patient sees it before paying.

Can we use patient weight loss results and testimonials in GoHighLevel campaigns?

Only with care. The FTC says a testimonial does not replace scientific evidence for the claim it implies, and that a "results not typical" line does not fix a dramatic result; the ad must show what a typical patient can expect (FTC Health Products Compliance Guidance). Using a patient's information in marketing may also need their written HIPAA authorization (HHS marketing guidance). Your counsel approves the claims; we build the pages to that approval.

Does GoHighLevel connect to our EHR or telehealth platform?

Sometimes, and it depends on the vendor. Where the platform offers an API, webhooks or a Zapier app, we connect it through n8n or a direct integration. Confirm first that the vendor will sign a BAA covering that data flow, and keep the clinical record in the EHR.

Do you set up GoHighLevel for clinics in any US state?

Yes. We build remotely for clinics across the US and in other countries. Autoesta is a 16-person team, about 8 in San Francisco and the rest in India and Nepal. State rules on prescribing, telehealth and advertising are set with your counsel; we build the system to match.
