Do appointment reminders reduce no-shows?
Yes, in published trials. A randomized controlled trial at a pediatric clinic found a no-show rate of 23.5% with text reminders against 38.1% without them. It is one small study, so use it as evidence of direction, not a promise for your business.
The trial enrolled 169 patients (84 control, 85 intervention) at an urban pediatric resident clinic and reported p = 0.04. The authors note limits: it covered infants and toddlers, they could not confirm every text was received, and 24% of patients still missed appointments with the reminders. The findings may not carry over beyond similar populations. Read the full study. We use it because it is a randomized trial, not because the numbers will match yours.
The lesson for a business is to measure your own baseline. A clinic, a salon and a consultancy face different no-show causes, and a reminder only fixes the forgetful ones.
What does a reminder sequence need?
A confirmation right after booking, one or more reminders before the visit, an easy way to confirm or reschedule, and a rule for what happens when there is no reply. Each message asks for one action.
| When | Message | Action asked |
|---|---|---|
| At booking | Confirmation with date, time, place and a calendar link | Add to calendar |
| 2 to 3 days before | Reminder with a reschedule link | Confirm or reschedule |
| Morning of, or 2 hours before | Short reminder with directions or a join link | Reply C to confirm |
| No confirmation | Alert to a staff member to call | Human follow-up |
| After a no-show | A kind message offering the next open slot | Rebook |
This timing is our starting design, not a proven optimum. Published evidence on the best number and timing of reminders is limited, so test two variants against your own no-show rate before you settle on one.
How do you build it in GoHighLevel?
Use a calendar with confirmations and reminders switched on, a workflow triggered by the appointment status, and two-way SMS so replies are read. Add a staff task when a confirmation does not arrive.
- Book in one calendar. Sync personal calendars so double-bookings do not create no-shows.
- Trigger on appointment status. Start the sequence when status is Confirmed, and stop it when the status changes to Cancelled or Showed.
- Handle replies. Read "C" to confirm and "R" to reschedule, and route anything else to a person.
- Create a task when no reply arrives. A short call from staff can close gaps that text does not.
- Log outcomes. Track Showed, No-show and Cancelled so you can measure the change.
See appointment booking automation for the service and dental appointment reminder software for a worked example in one industry. If your booking system lives elsewhere, an n8n workflow can pass appointment status into GoHighLevel.
What are the messaging rules?
Keep reminders minimal and give people a clear opt-out. Automated marketing texts need prior consent under the FCC's TCPA rules, and health-related reminders should avoid the reason for the visit. Ask counsel to review your wording.
For health practices, HHS treats appointment reminders as part of treatment and operations, and expects minimal content. Our guides on healthcare clinics and HIPAA and automation tools cover the details. This is not legal advice.
When will reminders not fix no-shows?
When people no-show for reasons a reminder cannot change: cost, fear, the wrong appointment type, or a long wait for the slot. Reminders fix forgetting, and other causes need other fixes.
- Long lead times. A long gap between booking and visit gives people more time to forget or change plans. Shorten the wait where you can, or add a mid-point check-in, and compare the result on your own data.
- Cost surprises. State the price and cancellation policy at booking.
- Unclear next step. If the message does not say where to go and how long it takes, people stay home.
- Nobody reads replies. Two-way texting only works if a person watches the inbox.
How do you measure whether it worked?
Measure the no-show rate as no-shows divided by appointments that were kept on the calendar, by appointment type, for the same length of time before and after. Track cancellations and reschedules separately, because a good sequence moves no-shows into early cancellations.
| Outcome | Why track it |
|---|---|
| No-show | The number you want to lower |
| Cancelled with notice | Should rise as no-shows fall, because the slot can be refilled |
| Rescheduled | Shows the reschedule link is being used |
| Confirmed by reply | Shows how many people engage with the reminder |
A fall in no-shows with a matching rise in cancellations is a good result, since a cancelled slot can go to someone else. Keep the trial's lesson in mind: even with reminders, 24% of patients in that study still missed their visit. Expect to lower the rate, not remove it. Our case studies show how we report outcomes on published builds.
How do you get started?
Count your no-show rate for a month, build the three-step sequence with a reschedule link, and compare after four weeks. Change one thing at a time.
- Record no-shows, cancellations and reschedules by appointment type.
- Build the confirmation and two reminders with a reschedule link.
- Add the staff task for unconfirmed appointments.
- Check consent wording and number registration.
- Compare no-show rate after four weeks, then test a variation.
Our GoHighLevel setup service builds this, and the aesthetic clinic guide shows a no-show flow for consultations.
More questions about no-shows
How many reminders should you send?
Published evidence on the best number is limited. Start with a confirmation and two reminders, and test one against three with your own data.
Should I charge a no-show fee?
That is a business and legal decision we do not advise on. If you use one, state it clearly at booking, and check the rules for your industry and state.
Do text reminders work better than email?
Trials of text reminders are common, and the one we cite tested text. Email suits confirmations and details, and text suits the last nudge. Use both and measure.
Can an AI agent call to confirm appointments?
Yes, and it is useful for people who do not reply to texts. Consent and disclosure rules apply. See our AI calling agent service.
