What should an appointment reminder text say?
A good appointment reminder text names your business, gives the date and time, offers a one-word way to confirm or a link to reschedule, and says how to opt out. Keep it under 160 plain characters so it sends as one SMS segment, and leave out anything private, such as the reason for the visit.
Below are nine appointment reminder text templates you can copy, each written twice: in plain words, and with the GoHighLevel merge fields that fill it in automatically. We counted every one. With sample values filled in, all nine fit in a single 160-character segment, which keeps them cheap and stops phones splitting them into pieces.
What this page adds: the character and segment count for each message, what one stray emoji or curly apostrophe does to your bill, and the rules from the FCC and HHS that decide what a reminder may contain. Sources were read on October 17, 2026.
Which appointment reminder text templates can you copy?
Use one template per moment in the booking: a confirmation when the appointment is made, a reminder a day ahead with reply-to-confirm, a short same-day reminder, and a follow-up if the person does not show. The table gives each one with its character count and segment count.
Characters in each template, against the 160-character limit
Counts use sample values: a 19-character business name ("Bright Smile Dental"), "Maria", "Oct 21", "2:30 PM" and a 25-character short link. Longer names or links change the count, so check yours with the method further down.
| Moment | Template | Characters | Segments |
|---|---|---|---|
| Booking confirmation | Hi Maria, you're booked at Bright Smile Dental on Oct 21 at 2:30 PM. To change it: [reschedule link] Reply STOP to opt out. | 131 | 1 |
| 24-hour reminder with reply to confirm | Reminder: Bright Smile Dental tomorrow at 2:30 PM. Reply C to confirm or R to reschedule. Reply STOP to opt out. | 112 | 1 |
| Same-day reminder | See you at 2:30 PM today, Maria. Bright Smile Dental, 120 Main St, San Mateo. Running late? Call (415) 555-0134. | 112 | 1 |

| Reply when they text C | Thanks Maria, you're confirmed for 2:30 PM. See you then. | 57 | 1 |
| Reply when they text R | No problem. Pick a new time here: [reschedule link] | 59 | 1 |
The meeting link in our count was 33 characters; booking and reschedule links were 21 to 25. Every template uses straight apostrophes. That detail matters, as the next sections show.
How do you write these templates with GoHighLevel merge fields?
In GoHighLevel, swap the sample values for merge fields such as {{contact.first_name}}, {{appointment.only_start_date}}, {{appointment.only_start_time}} and {{appointment.reschedule_link}}. They only fill in when the workflow starts from an appointment trigger, so test with a real booking.
The field names below come from HighLevel's list of custom values and merge fields. HighLevel's appointment custom values article adds the rule that trips people up: the appointment fields only show up when the workflow uses the Appointment or Customer Booked Appointment trigger. A pipeline stage trigger does not know which appointment you mean, so the fields come out blank.
| Template | GoHighLevel version |
|---|---|
| Booking confirmation | Hi {{contact.first_name}}, you're booked at {{location.name}} on {{appointment.only_start_date}} at {{appointment.only_start_time}}. To change it: {{appointment.reschedule_link}} Reply STOP to opt out. |
| 24-hour reminder | Reminder: {{location.name}} tomorrow at {{appointment.only_start_time}}. Reply C to confirm or R to reschedule. Reply STOP to opt out. |
| Same-day reminder | See you at {{appointment.only_start_time}} today, {{contact.first_name}}. {{location.name}}, {{location.full_address}}. Running late? Call {{location.phone}}. |
| Healthcare reminder | {{location.name}}: reminder of your appointment on {{appointment.only_start_date}} at {{appointment.only_start_time}}. Questions? Call {{location.phone}}. Reply STOP to opt out. |
| Online meeting | Your call with {{appointment.user.name}} starts at {{appointment.only_start_time}}. Join here: {{appointment.meeting_location}} Reply STOP to opt out. |
| Reply to R | No problem. Pick a new time here: {{appointment.reschedule_link}} |
Two more fields are worth knowing: {{appointment.timezone}} for clients in other time zones, and {{appointment.cancellation_link}} if you want a cancel option. The C and R replies need a workflow with a "Customer Replied" trigger and a filter on the reply text, which then updates the appointment status or sends the reschedule link.
Why does one emoji make an appointment reminder text cost more?
One emoji or curly apostrophe switches the whole message from GSM-7 to UCS-2 encoding, which cuts a single segment from 160 characters to 70. Our 131-character confirmation then sends as two segments, so it costs twice as much and some phones show it in two parts.
SMS segment sizes
The limits come from the SMS standard, as Twilio's SMS character limit page sets out. HighLevel's phone system bills per segment, so they apply to every text it sends:
| Encoding | When it is used | One segment | Each segment of a longer message |
|---|---|---|---|
| GSM-7 | Plain letters, numbers and common punctuation | 160 characters | 153 characters |
| UCS-2 | Any emoji, curly quote or non-Latin character | 70 characters | 67 characters |
The usual culprit is copying a template from Word or Google Docs, which turns a straight ' into a curly ’ and " into “. Type templates straight into GoHighLevel, or paste them into Twilio's free segment calculator, which flags the characters that force UCS-2.
How much do appointment reminder texts cost to send?
On GoHighLevel's phone system, a one-segment US text costs $0.00747 plus a carrier fee of about $0.0035 to $0.0050, at the time of writing. That is roughly $11 to $12.50 per 1,000 one-segment reminders. A message that spills into two segments doubles the base charge.
Base cost per 1,000 texts by segments
Rates are from HighLevel's Phone System pricing guide, read October 17, 2026: $0.00747 per segment for US SMS, outbound and inbound, and outbound carrier fees of $0.0035 (AT&T), $0.0045 (T-Mobile) and $0.0050 (Verizon). Inbound replies such as "C" are billed at the same per-segment rate.
A worked example, labelled illustrative: a clinic with 500 appointments a month sends three texts per appointment (confirmation, 24-hour, same-day), so 1,500 messages.
| Messages sent as | Base charge (1,500 x $0.00747 per segment) | Carrier fees | Monthly total |
|---|---|---|---|
| 1 segment each | $11.21 | $5.25 to $7.50 | $16.46 to $18.71 |
| 2 segments each (one curly apostrophe) | $22.41 | $5.25 to $7.50 | $27.66 to $29.91 |
The carrier fee rows assume one fee per message; check how your carrier mix bills longer messages. The cost is small either way next to one missed appointment, which our no-show research puts in context.
Are appointment reminder texts HIPAA compliant?
Yes, if you limit what they say, because HHS says the HIPAA Privacy Rule lets providers send reminders but expects them to limit the details, such as the practice name, a number and what is needed to confirm. Leave out the treatment, the provider's specialty and any results.
HHS's answer is in its HIPAA FAQ on leaving messages for patients (content last reviewed December 28, 2022). It was written about answering machines and family members, and the same caution applies to a text that anyone holding the phone can read. Compare these two:
- Too much: "Hi Maria, this is Bright Smile Dental. Your teeth whitening appointment with Dr. Lee is on Oct 21 at 2:30 PM. Please arrive 10 minutes early. Reply STOP to opt out." That is 164 characters, two segments, and it names the treatment.
- Enough: the healthcare template above, at 126 characters, with the practice name, date, time, a number and STOP.
HHS also says that if a patient asks to be contacted another way, the practice must accommodate a reasonable request. Store that preference on the contact so the workflow respects it. The tool side, which platforms will sign a business associate agreement, is covered in our guide to HIPAA-compliant automation.
What rules apply to automated appointment reminder texts?
In the US, automated reminders need the person's consent to be texted, a working opt-out, and a registered sending number. Reminders are informational, not marketing, so keep promotions out of them. Healthcare providers have an extra FCC exemption with strict limits on length and frequency.
The healthcare exemption is in the FCC's 2015 order FCC 15-72 (adopted June 18, 2015), paragraphs 146 to 148. It covers appointment and exam confirmations and reminders, among other healthcare messages, and only when they are free to the recipient, including not counting against their plan limits. The conditions read like a checklist for a good reminder in any business:
- Send only to the wireless number the patient provided.
- State the provider's name and contact information.
- No telemarketing, advertising, billing or debt collection content, and follow HIPAA.
- Keep texts to 160 characters or less (voice calls about one minute).
- No more than one message a day, and three a week, from one provider.
- Tell people they can reply STOP to opt out.
- Honour opt-outs immediately.
Outside that exemption, the normal consent rules apply. A STOP to a reminder must be honoured; the FCC's 2026 order changes how far a STOP reaches, which our FCC opt-out news note explains. Send inside normal hours (our page on what time businesses can text customers covers state rules), and register your number for A2P 10DLC first: our registration guide shows how. This is general information, not legal advice.
Do appointment reminder texts reduce no-shows?
Yes, a 2013 Cochrane review found attendance of 78.6% with text reminders against 67.8% with none, across seven studies. Texts worked about as well as phone call reminders and cost less per attendance. The templates only matter if they reach people at the right times.
The figures are from Gurol-Urganci and colleagues in the Cochrane Database of Systematic Reviews, summarised with the other studies on our appointment no-show statistics page. For timing, our appointment booking automation page sets out the full schedule we build, from confirmation to the "we missed you" text, and our reduce no-shows page covers deposits and confirmation calls for high-value appointments.
How do you send appointment reminders by text automatically?
Connect your booking calendar to a CRM or texting tool, start a workflow when an appointment is booked, and send each template at a set time before the start time. In GoHighLevel that is a workflow with an appointment trigger, wait steps timed against the appointment, and a reply branch for C and R.
- Register the number. A2P 10DLC for local numbers, or toll-free verification.
- Collect consent at booking. A separate, unticked checkbox for reminder texts on the booking form.
- Trigger on the appointment. Use the Appointment or Customer Booked Appointment trigger so merge fields fill in.
- Time each message. Wait until a set time before the appointment start, then send; skip steps when the appointment is less than that far away.
- Handle replies. C marks the appointment confirmed; R sends the reschedule link; STOP is handled by the platform.
- Stop on change. Cancelled or rescheduled appointments must leave the old sequence.
- Test with a real booking on a team phone and read every message as the patient or customer would.
Questions people ask about appointment reminder texts
How many days before an appointment should you send a reminder text?
A common schedule is a reminder one to two days before, with a reply to confirm, and a short reminder two to three hours before. For appointments booked weeks ahead, add one more a few days out. Healthcare providers relying on the FCC exemption must stay within one message a day and three a week.
Should appointment reminder texts include a confirm option?
Yes, when you can act on the answer. "Reply C to confirm or R to reschedule" tells you who is coming and frees slots early enough to refill them. It only helps if a workflow reads the reply and updates the appointment, so build that before you add the line to the template.
Can I send appointment reminders without written consent?
Reminders are informational messages, which carry lighter consent requirements than marketing, and a customer who gives their number when booking has usually agreed to messages about that booking. Keep the reminder free of promotions, include STOP, and record the consent on the contact. Check state rules and get legal advice for your situation.
What should a missed appointment text say?
Keep it short and friendly, with no blame and no fee threat in the first message: "Hi Maria, we missed you today at Bright Smile Dental. Pick a new time here: [link] Reply STOP to opt out." Send it within an hour of the missed slot, while the person still remembers. Mention any cancellation policy separately if you have one.
Can I add emojis to appointment reminder texts?
You can, but one emoji switches the message to UCS-2, where a segment holds 70 characters instead of 160. A 130-character reminder becomes two segments and costs twice as much. If you want an emoji, keep the whole message under 70 characters, or leave it out.
Can appointment reminders go to a landline?
Not by SMS: most landlines cannot receive texts, and the message fails or is billed with nothing delivered. Mark landline numbers on the contact and send those people an email or an automated call reminder instead. HighLevel offers a number validation check before sending, at $0.005 per check at the time of writing.
Should reminders come from the same number customers call?
Yes, where possible, because people reply to the number they recognise and a reminder from your main business number is less likely to be ignored as spam. In GoHighLevel, set that number as the default outbound number and make sure it is registered for texting.
